Do these requirements extend to SBM plans, or just FFM?Do we need to include MSP plans?

All issuers must publish an up-to-date, accurate, and complete list of all covered drugs and provider directories for the 2016 plan year (see 45 CFR 156.122(d), 156.122(d)(1), and 156.122(d)(2), for formulary requirements and 45 CFR 156.230(b), 156.230(b)(1) and 156.230(c) for provider requirements.This specific HHS required machine-readable format discussed here applies to any issuers using the FFM, per 156.122(d)(2) and 156.230(c).
